Job Description
The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team, collaborating with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/safe discharge plans.
Requirements
Associate's Degree in Nursing required
2 or more years of healthcare experience preferred
Case Management or related experience preferred
Current RN licensure in state of employment required
Weekend and holiday coverage
Knowledge/Competence of Microsoft Office products and keyboarding
