Role OverviewThis position is for Discharge Planning in Case Management at a hospital in Lexington, KY. Case managers work with physicians, nurses, physical therapists and other members of the health care team to identify and coordinate alternative care or services that will assist in recovery following discharge. The role is full-time, days.
What You Will Do
Assess appropriateness of admission and determine correct patient status using payer criteria and clinical assessment. Screen all patients to identify the need for case management services, gather relevant information by interviewing the patient/family and healthcare team, and prioritize needs based on patient goals. Evaluate treatment plans to identify barriers, engage the patient/family as primary decision makers, and educate them to make informed decisions. Document in Case Management systems and the medical record.
