Role OverviewThe Case Manager (Specialist) provides intensive case management services to eligible clients, meeting participants at least weekly. The Case Manager is the primary point of contact to coordinate services and care for assigned clients.
What You Will Do
Conducts comprehensive screenings and assessments, provides service linkages and support systems, coordinates individualized planning, and acts as an advocate for clients.

Requirements
- Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
- Ability to perform under circumstances of possible emotional stress and conflicts dealing with difficult, uncooperative, and potentially aggressive clients
- Associate’s Degree in Social Work or related field or a combination of education and relevant experience (Two years’ total)
- Completion of VOALA Case Management training within 6 months of hire
- Minimum 1-year general experience in case management or similar social services role
