Job Description
We are seeking a Case Manager to provide case management services to people experiencing homelessness in order to assist them in transitioning to permanent housing.

Requirements

  • Provide direct services to a caseload of approximately 15 clients
  • Conduct psychosocial evaluations and connect clients to psychiatric services
  • Assist clients in obtaining permanent housing and maintain record keeping in compliance with agency standards
