Job Description
Case Manager, Transitional Housing provides case management services to people experiencing homelessness to assist them in the transition to permanent housing. The role involves conducting assessments, connecting clients to psychiatric services, medical treatment, and substance use treatment, and assisting clients in obtaining permanent housing.

Requirements

  • Bachelor's Degree or equivalent work experience
  • Two years of experience working with homeless adults and substance abuse issues
  • Experience with harm reduction and motivational interviewing techniques
  • Valid NY, NJ, CT, or PA driver's license with a good driving record
  • Excellent computer skills, including proficiency in Microsoft Word and Excel
  • Excellent writing and communication skills
  • Bilingual Spanish/English
