Job Description
Role Overview

The Case Planner provides family-focused practice to promote permanency, safety, and well-being, and to reduce length of stay in foster care. They provide comprehensive casework services for both youth and families, and ensure that all ACS mandates are in compliance.

What You Will Do

The Case Planner will maintain primary case planning responsibility for 16-20 cases, conduct required casework contacts with the child, parent, and collateral contacts, and work collaboratively with the Social Worker to make assessment-based referrals.

Requirements

  • Bachelor's degree required
  • Experience working with families and adolescents in the child welfare system required
  • Must be able to establish working relationships with participants in need of supportive services
  • Must have working knowledge of Connections
  • Must have excellent verbal and written communication skills
  • Ability to work flexible hours, including two evenings
  • Ability to use public transportation to travel throughout New York City
  • Must have a commitment to GSS models of practice including Sanctuary and ACS principles to promote permanency, safety and well-being
  • Must have a commitment to work from a strength-based, youth development and trauma informed perspective
  • Bilingual English/Spanish preferred
  • NYS Drivers Licensed preferred
