Role OverviewThe Case Worker Health Educator provides specialized educational counseling services within an assigned functional area. They communicate information to clients regarding program services, follow established procedures, and receive general guidance and direction. The role requires originality and ingenuity to locate, select, and apply appropriate procedures, processes, and techniques to assignments that are broad in nature.
What You Will Do
The Case Worker Health Educator will provide advocacy, culturally appropriate health education, and information regarding assigned program objectives to clients and communities. They will also provide care coordination, case management, and system navigation activities, as well as conduct outreach, document services, and report on program progress.

They will also have skills in communication, instructing, and operating standard office equipment. Requirements
- Associate degree
- At least two years of experience in sales, marketing, public health, social work, or human services
- Certification as Diabetes Prevention Lifestyle Coach or obtained within 3 months of hire (if employed in the diabetes program)
