Role OverviewThe Case Worker – Health Educator provides specialized educational counseling services within an assigned functional area. They communicate information to clients regarding program services, follow established procedures, and receive general guidance and direction. The role requires originality and ingenuity to locate, select, and apply appropriate procedures, processes, and techniques to assignments that are broad in nature.
What You Will Do
The Case Worker – Health Educator will provide advocacy, culturally appropriate health education, and information regarding assigned program objectives to clients and communities. They will conduct outreach, document services, report on program progress, and create educational/program marketing materials.

The ideal candidate will have knowledge of community resources, basic health and wellness principles, and basic EMR functionality. They will be skilled in communicating effectively, both orally and in writing, and will be able to partner with other functional areas to accomplish objectives.
Requirements
- Associate degree
- At least two years of experience in sales, marketing, public health, social work, or human services
- Chauffeur's License preferred but not required
- Certification as Diabetes Prevention Lifestyle Coach or obtained within 3 months of hire
