Job Description
Metropolitan Family Services is seeking a Targeted Case Management Specialist to join their team. The ideal candidate will be responsible for providing community targeted case management to CCBHC clients, gathering information for the IM+CANS, and developing specific goals integrated into the IATP. The successful candidate will be a team player who is passionate about social justice and advocacy, with a commitment to empowering families to reach their greatest potential.

Requirements

  • Bachelor's degree in a human services field or 2+ years' experience in the mental health field, social service or healthcare setting, required.
  • Must obtain and maintain IM+CANS certification, required.
  • Bilingual in Spanish, preferred.
