What You Will Do
Manage PCP's and NP's, patient population: seniors with 3-5 chronic diseases, value based care, and leadership responsibilities.

Requirements
- Board Certified in: FM, IM or Geriatric Medicine
- Education: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Experience: At least 1 year of clinical experience in a geriatric or family practice setting. Leadership experience required.
- Skills: Strong leadership, communication, and analytical skills. Proficiency in Microsoft Office and the ability to travel locally and occasionally regionally.
