Requirements
- Valid HHA or PCA Certification
- Valid ID (Driver's License, Passport, or State ID)
- Social Security Card
- Employment Authorization Card (if required by your Social Security card)
- Pre-Employment Physical Form - must include MMR, TB test, and Drug Screen
- COVID-19 Vaccine Card (if you received it)
- Two (2) Work-Related References
