Role OverviewUnder the direction of the Paraprofessional Team Leader, performs assigned duties in the patient’s home in a safe and considerate manner as prescribed by the case manager.
What You Will Do
Maintains open communication with case manager and/or Team Leader/Assistant Team Leader regarding health, safety and environment status of patients, reports all changes in a timely manner, and completes and submits required documentation.

Requirements
- Certification as a Home Health Aide or equivalent
- One year satisfactory employment history
- Experience in care of the elderly
