Role OverviewPurposeCare is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our team as a Change in Condition (CIC) Team Lead. In this role, you'll serve as a vital connection between our Home Care and Home Health teams by reviewing changes in patient conditions, identifying opportunities for skilled home health services, and coordinating care to ensure patients receive the right services at the right time.
What You Will Do
Review Change in Condition (CIC) forms submitted by Home Health Aides. Identify patients who may benefit from skilled home health services. Coordinate referrals with the Clinical Liaison and Home Health Intake Coordinator.

Requirements
- Current Licensed Practical Nurse (LPN) license in the state of practice
- Graduate of an accredited nursing program

- Ability to work independently while collaborating with interdisciplinary teams
- Strong organizational and documentation skills
