Role OverviewMerakey is seeking a Case Manager to join their Children's Behavioral Health Program serving clients in New Castle County, Delaware. The Case Manager will coordinate resources and information pertinent to client needs, document all coordination of resources, and ensure client charts are up-to-date.
What You Will Do
The Case Manager will coordinate referrals for client specific discharge planning, monitor record keeping and compliance, and complete necessary documentation of each case. Case sessions are primarily delivered in the community, and case management can be in an office environment or community.
Why It Might Be a Fit
The ideal candidate will possess a Bachelor's degree, at least 1 year of experience working with children/adolescents, a valid Driver's License, and a clean driving record. The candidate should be able to work Monday-Friday with an ON-CALL rotation.
