Job Description
Chronic Disease Specialist will work with health plans, providers, and patients to identify and address open care opportunities, schedule appointments, and provide education and support for disease prevention and health maintenance.

Requirements

  • Collaborate with health plans, hospitals, and provider teams
  • Contact and schedule appointments for patients with open care opportunities
  • Data collection and reporting on patients with a recent hospitalization or emergency department visit
  • Document patient encounters in the Electronic Health Record
  • Maintain a current list of community agencies and resources
  • Assist provider teams in completing required documentation of care
  • Participate in Quality, Compliance, and Infection Control Programs
