Job Description
The Clinical Coordinator provides administrative oversight and support to the treatment team, provides direct care to adults living with severe and persistent mental illness and substance use disorders, and links recipients to services to obtain housing and achieve wellness self-management and increasing independence.

Requirements

  • Master's level degree in human services field with appropriate NYS licensure
  • Previous work on an ACT team or similar type of community based/mobile based program for at least one year
  • Ability to read data and documents, including spreadsheets, reports and Electronic Health Records in printed form and on computer screens
