Requirements

  • Must be licensed at the associate or independent level with the Arizona Board of Behavioral Health Examiners (AZBBHE) as Psychologist, Social Worker, Counselor or Marriage and Family Therapist
  • Valid identification
  • AZ Fingerprint Clearance Card
