Job Description
Hopscotch Primary Care is seeking a Clinical Documentation Specialist (CDS) to support patient care across the Western North Carolina region. This role involves comprehensive chart reviews, preparation, and collaboration with clinic leaders. The CDS will partner with the team to support daily huddle meetings and monthly review processes. This is a remote position focused on supporting care teams and ensuring efficient medical records and HIE workflows.

Requirements

  • Chart preparation in advance of scheduled visits
  • Full chart builds for new patients
  • Access and review HIE/EMRs to identify potential suspects based on historical chart data
  • Collaborate with population health team to improve provider documentation
  • Monitor clinical documentation efforts
  • Proficiency in tracking down and leveraging external resources
