Job Description
Clinical Nutritionist position at Ryan Health to provide nutrition assessment and medical nutrition therapy to patients and serve as a resource for staff and community.
Requirements
Provide medical nutrition therapy to patients
Assess patient’s nutritional status
Develop individualized and culturally competent nutrition care plans
Document nutrition assessment, care plans, and other details in the patient’s electronic medical record
Refer nutrition patients to other Ryan Health services or community-based agencies
Participate in care-team meetings and interdisciplinary case conferences
Develop and facilitate group self-management and education projects
Conduct peer and chart reviews and other quality improvement activities
