Role OverviewThis position provides a broad scope of clinical practice, combining direct patient care, collaborative disease state management, interdisciplinary teamwork, teaching and precepting, and community and professional engagement. The ambulatory clinical pharmacist will provide comprehensive medication and disease state management across a diverse range of clinical areas.
What You Will Do
Provide comprehensive medication and disease state management across a diverse range of clinical areas, including heart failure, hyperlipidemia, anticoagulation, diabetes, COPD/asthma, weight management, and transitions of care. Collaborate with physicians, nurse practitioners, pharmacists, nurses, and other members of the healthcare team to optimize medication therapy and improve patient outcomes.

Requirements
- Doctor of Pharmacy degree from a college or school of pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE) preferred
- Bachelor’s degree from an ACPE-accredited college or school of pharmacy with demonstrated significant clinical pharmacy experience and training
- Eligibility for licensure as a pharmacist in Ohio
- Current Basic Life Support (BLS) certification
- American Heart Association Advanced Cardiovascular Life Support (ACLS) certification recommended
- Completion of an ASHP-accredited PGY1 pharmacy residency strongly preferred
- Experience in ambulatory care pharmacy required
