Role OverviewThe Clinical Resource Manager is responsible for facilitating and coordinating patient care for a specific patient population or area of assignment. They work with the interdisciplinary team to plan and implement individualized safe and appropriate discharge plans, promoting high-quality, efficient, and cost-effective care.
What You Will Do
Assess patient needs, plan and implement discharge plans, and facilitate care that meets established patient care needs while optimizing reimbursement.
Why It Might Be a Fit
Must have at least 5 years of clinical experience as a Registered Nurse, experience with utilization review and discharge planning, and be PRI and Screen certified.
Requirements
- Bachelor's degree
- Current registration in New York State as an RN
- At least 5 years of clinical experience as a Registered Nurse
- Experience with utilization review and discharge planning
- PRI and Screen certification
- Interqual knowledge/experience
- Knowledge of Microsoft Word and Excel
