Job Description
The Transitions of Care Clinical Staff Pharmacist - Transplant is responsible for ensuring patients are prepared for transitions of care and have the necessary information to take their medications safely and effectively.
Requirements
Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy
Good general health and stress-coping ability
Licensure as a pharmacist by the Texas State Board of Pharmacy
