Job Description
The Transitions of Care Clinical Staff Pharmacist - Transplant is critical in ensuring patients are prepared to be transitioned to different levels of care and have the information necessary to take their medications safely, appropriately, and consistently.

Requirements

  • Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy required.
  • Good general health and stress-coping ability.
  • Licenses and Certifications: (PHARMD) DOCTOR OF PHARMACY Upon Hire, Licensed Pharmacist by Texas State Board of Pharmacy (TSBP) Upon Hire, (BLS) BASIC LIFE SUPPORT certification with a course accredited by the American Heart Association (AHA) Upon Hire
