Job Description
The Transitions of Care Clinical Staff Pharmacist - Transplant is responsible for ensuring patients are prepared for transition to different levels of care and providing medication counseling and education.

Requirements

  • Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy required.
  • Good general health and stress-coping ability.
  • Licensure as a pharmacist by the Texas State Board of Pharmacy (TSBP) required.
  • BLS certification with a course accredited by the American Heart Association (AHA) required.
