Job Description
The Clinical Staff Pharmacist, Transitions of Care - Transplant is responsible for ensuring patients are prepared for transition to different levels of care and have the necessary information to take their medications safely and effectively. The pharmacist will review medications, provide counseling, and address discrepancies or recommendations with the patient's primary team or prescriber.

Requirements

  • Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy required.
  • Good general health and stress-coping ability.
  • Licenses and Certifications: (PHARMD) DOCTOR OF PHARMACY Upon Hire, Licensed Pharmacist by Texas State Board of Pharmacy (TSBP) Upon Hire, (BLS) BASIC LIFE SUPPORT certification with a course accredited by the American Heart Association (AHA) Upon Hire
