Job Description
The Clinical Staff Pharmacist, Transitions of Care position is responsible for ensuring patients are prepared for transitions of care and have the necessary information to take their medications safely and effectively. The pharmacist will review medication orders, provide counseling, and participate in specialty consults to support patient care.

Requirements

  • Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy required
  • Licensed Pharmacist by Texas State Board of Pharmacy (TSBP) Upon Hire
  • BLS certification with a course accredited by the American Heart Association (AHA) Upon Hire
