Job Description
The Transitions of Care Clinical Staff Pharmacist is responsible for ensuring patients are prepared for transitions to different levels of care and have the necessary information to take their medications safely and effectively.

Requirements

  • Doctor of Pharmacy (PharmD) degree or equivalent
  • Licensure as a Pharmacist by the Texas State Board of Pharmacy
  • Basic Life Support (BLS) certification
