Job Description
The Clinical Staff Pharmacist, Transitions of Care is responsible for ensuring patients are prepared for transitions of care and have the necessary information to take their medications safely. The pharmacist will review medication orders, provide counseling, and address discrepancies with the primary team or prescriber.
Requirements
Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school of pharmacy
Licensure as a pharmacist by the Texas State Board of Pharmacy
BLS certification by the American Heart Association
