Job Description
The Transitions of Care Clinical Staff Pharmacist is critical in ensuring patients are prepared to be transitioned to different levels of care and have the information necessary to take their medications safely, appropriately, and consistently.

Requirements

  • Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy required.
  • Good general health and stress-coping ability.
  • Licensed Pharmacist by Texas State Board of Pharmacy (TSBP) Upon Hire
  • BLS) BASIC LIFE SUPPORT certification with a course accredited by the American Heart Association (AHA) Upon Hire
