Job Description
The Clinical Transformation Specialist will work with Atlantic Health Group to implement clinical workflows and improve quality measures. The role will require experience in clinical practice, care management, and eCQM programs, as well as the ability to drive clinical workflow change with frontline providers and clinical staff.
Requirements
Active clinical licensure (RN, NP, PA, LCSW, or equivalent clinical credential)
3+ years of experience in clinical practice, care management, population health, or ambulatory clinical operations
Direct experience with eCQM, HEDIS, or CMS quality measure programs
Experience working with Epic or comparable EHR systems in a multi-site ambulatory environment
