Job Description
The Community Adjustment Trainer provides professional case management services for outpatient clients, assisting them to function independently in self-help and daily living skills. The position involves delivering and monitoring medication compliance, supporting clients with daily needs, and collaborating with family members and other service providers in the community.

Requirements

  • 6 months of training or experience in supporting and assisting consumers who are in forensic legal status
  • Valid driver's license
  • 6 months of experience or training in providing support or assistance with personal care or Activities of Daily Living (i.e. ADL)
  • Valid driver's license
