Role OverviewThe ACCS Community Connector will work to ensure successful community integration by connecting adults with psychiatric conditions with available community resources. The role involves supporting Person Served as citizens living within communities and ensuring access to community resources and social networks. The Community Connector will provide information about community resources and entitlements/benefits and support Person Served in preparing for, seeking and keeping wanted employment and/or educational opportunities.

Provide interventions to support Person Served in preparing for, seeking and keeping wanted employment and/or educational opportunities. Connect adults with psychiatric conditions with available community resources to promote successful integration into their communities.

Requirements
- Bachelor's degree in human services, Master's degree in Human Services, or very experienced direct care staff
- 2 to 3 years of experience working with adults with psychiatric conditions
- Access to an operational and insured vehicle and willingness to use it to transport Person Served
- Ability to pass a CORI, driving record, reference, educational and professional licensure check as well as a multi-state background check
- Ability to become CPR, First Aid certified within 2 weeks of employment and MAP certified within 150 days of employment
