Role OverviewJoin Scripps Health as a Community Health Promoter supporting the AHEC department at Scripps Mercy Hospital Chula Vista. As a Community Health Promoter II, you'll create a positive Scripps Health experience for our patients while being responsible for various tasks.
What You Will Do
Primary responsibilities include following up with discharged patients, providing community health presentations, assisting with support groups, and providing data entry support. You'll also identify patients from the emergency department who qualify for Enhanced Case Management and connect them to local partner sites.

The ideal candidate will have excellent communication skills, experience working independently and as a team player, and the ability to set priorities and goals without specific direction. Bilingual in Spanish and basic typing and computer skills are preferred qualifications.
Requirements
