Role Overview

The Community Health Worker (CHW) will work with Enhanced Care Management (ECM) patients to improve their whole health through outreach and engagement activities. This position will serve as an integral component of the Street Medicine program, engaging and enrolling individuals experiencing unsheltered homelessness into ECM.

What You Will Do

The CHW will conduct community outreach, establish trusting relationships with patients, and provide ongoing follow-ups to an assigned caseload of patients. They will also assist patients in navigating the healthcare system, document patient barriers, and communicate with providers to improve care and outcomes.

Why It Might Be a Fit

The ideal candidate will have experience working with unsheltered homeless and/or personal lived homelessness experience, as well as knowledge of cultural issues, trauma-informed practices, and mental health conditions. They will be able to perform intermediate computer and data entry skills, engage patients in a variety of settings, and develop relationships with community patients and leaders.

Requirements

  • High school/GED required
  • Current Basic Life Support (BLS) certification
  • Two years’ healthcare, behavioral health, street medicine, or social services industry experience
  • Valid CA Driver’s License and proof of auto insurance
  • Experience working with unsheltered homeless and/or personal lived homelessness experience preferred
  • Bilingual (English/Spanish) preferred
