Role Overview

As the Community Health Worker, you will provide Healthy Start Program services to at-risk families by getting involved in the community and sharing resources and opportunities available to support their growing family journey. You will engage in outreach and recruitment efforts while enrolling families into the Healthy Start Program, team case management, screen health and safety questionnaires, review health information, and seek community referrals to assist at-risk families of South Phoenix and surrounding communities.

What You Will Do

You will work outside of the office to meet with families, recruit and enroll eligible families into Healthy Start Program services, and assist in scheduling families with a case manager. You will also collaborate with participants, partner agencies, and providers in the community over the phone and in person to ensure that health care and social service needs are being met.
