Role OverviewWe are looking for a dedicated candidate to join our team as a Community Health Worker (CHW) to service our patients in the Columbia area. The CHW serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality-of-service delivery.
What You Will Do
Deliver medications and durable medical equipment to patients, facilitate visits with providers and specialty services, and coordinate telehealth visits between patients and healthcare professionals.

Requirements
- Certified Medical Assistant, Paramedic, or Licensed Nurse preferred
- Phlebotomy experience preferred
- Previous experience in public health or home health preferred
- Experience with DME delivery or logistics operations preferred
- Must be proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records
- Strong organizational and time management skills
- Ability to read and communicate effectively
- Basic computer knowledge
- Must be able to work independently with minimal supervision and direction
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working condition
