Role OverviewWe're seeking an experienced Community Health Worker to provide care coordination and support services to underserved populations. You'll work with individuals enrolled in enhanced care management programs, helping them navigate healthcare, housing, and social services while addressing barriers like homelessness, chronic illness, and behavioral health challenges.
What You Will Do
Conduct community outreach, enroll members into managed health plans, manage a caseload, coordinate referrals, conduct home and community visits, connect members with resources, advocate for individuals, and maintain accurate documentation.

Requirements
- Minimum 2 years of professional experience in community health, outreach, care coordination, or related field

- Must live within or near the assigned service area
