Role OverviewThis role operates in a Day Habilitation (DAY HAB) Setting, providing opportunities and support for community inclusion and building interest in and developing skills for active participation in recreation, volunteerism, and integrated community employment. The Community Integration Professional will collaborate with individuals to identify interests, strengths, and goals, and develop meaningful daily and weekly schedules that include community participation, hobbies, volunteer work, and cultural, civic, or religious activities.
What You Will Do
The Community Integration Professional will be responsible for Person-Centered Community Engagement, Goal-Based Skill Development, Behavioral Support Implementation, Documentation & Progress Tracking, Safety & Advocacy, Vocational & Community Employment Support, and Program Participation & Professional Responsibilities.
