Role OverviewEnhabit Home Health & Hospice seeks a Community Liaison to serve as the primary relationship manager between the company and referral sources, supporting referral growth and providing awareness and education of company services and program requirements.
What You Will Do
The Community Liaison will provide awareness and education of company services and program requirements to referral sources, patients, families, caregivers, and community partners to help navigate post-acute care transitions and improve patient outcomes.

Requirements
- Bachelor's Degree in healthcare administration, marketing, or a related field
- graduate of an approved school of nursing, therapy, or social work
- 2 years of experience in healthcare business development, community relations, marketing, or sales
- demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations
- valid state driver license
- automobile liability insurance
- strong interpersonal, communication, and presentation skills
- clinical license (if applicable)
- experience in home health, hospice, post-acute care, or related healthcare services
- established relationships with physicians, hospitals, or senior living communities and other referral sources
- intermediate technology skills, including Microsoft Office
