Job Description
Thomas Allen is seeking a Community Living Counselor to assist and encourage individuals to realize their goals and promote their emotional, physical, and personal well-being.

Requirements

  • Meet with supervisor as needed
  • Assist individuals with meal preparation and daily living activities
  • Maintain up-to-date knowledge of each individual's service plan
  • Log all day-to-day occurrences to track progress
  • Accompany individuals to medical appointments and community activities
  • Administer and log medications as needed
  • Help individuals maintain their homes
  • Perform other duties as assigned
