Job Description
The Complex Care Coordinator – ADRD Focus position is responsible for researching, developing, and implementing a complex care management initiative with a focus on individuals living with brain injury, Alzheimer’s disease and related dementias (ADRD). The position will serve as a lead case manager and report to the Senior Long Term Care Division.

Requirements

  • Bachelor’s degree in health care, human services, business, or public administration
  • Four years’ experience in case management, including complex cases
  • Knowledge of Brain Injury, Alzheimer's disease and related dementias, person-centered care principles, and care models
  • Knowledge of program development, policy drafting and evaluation
  • Knowledge of regulatory requirements for Category D assisted living facilities
