Role OverviewThe Conference Facilitator will facilitate family team conferences to ensure the safety and well-being of children and families. The role involves working with key stakeholders, children, families, and service providers to make recommendations for referrals and resources. The candidate will also prepare administrative functions, including documentation and reports.
What You Will Do
Attend and facilitate family team conferences, address critical issues, and make recommendations for referrals and resources. Prepare administrative functions, including documentation and reports. Interface with interagency critical support resources and provide case conferencing and consultation.

The successful candidate will possess excellent written and verbal communication skills, analytical, organizational, and interpersonal skills. Proficient knowledge in the use of NYS Connections Database, Microsoft Word, Excel, and Access is preferred.
Requirements
- Masters Degree in Social Work from an accredited school of social work
- Valid New York State Registration as a Licensed Clinical Social Worker (LCSW) or Licensed Master of Social Work (LMSW)
- At least one year of full-time, satisfactory, post-graduate experience in facilitation of family team conferences or family group decision-making meetings
- At least three years of experience in clinical group work with children, youth, parents, or individual families
