Job Description
The Admissions Coordinator for Home Health Care supports patient outcomes through compassionate care, seamless transitions, and strong community partnerships. This role ensures patients receive timely, appropriate, and well-coordinated post-acute services that support recovery, safety, and continuity of care.

Requirements

  • Bachelor of Science in Nursing (BSN) preferred
  • ASN required
  • Minimum of two (2) years of experience in a healthcare setting required
  • Experience in Home Health or community health preferred
  • Current Registered Nurse (RN) license in the State of Maryland OR RN license from a Nurse Licensure Compact state with a Multi-State Privilege designation
  • Basic computer skills required
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)
  • Meditech experience preferred
  • Experience with care management software preferred
  • Excellent communication and interpersonal skills
  • Public speaking and patient education experience preferred
  • Knowledge of Medicare coverage criteria preferred
