Job Description
MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs, allowing seniors to obtain quality healthcare while remaining active and involved in the community.

Requirements

  • Provide hands-on support to older adults and individuals with brain injuries, intellectual/developmental disabilities, and behavioral or mental health needs
  • Assist with daily living activities (ADLs)
  • Transport and pick up program participants using the program van
  • Help plan and lead fun, engaging group and individual activities
  • Promote independence, self-esteem, and social skills
  • Ensure safety, care, and emotional support at all times
