Role Overview

The DDS Case Manager will coordinate and perform case management for residential program participants and other participants who receive funding/service provided by Dept. of Developmental Services (DDS). The role involves developing, implementing, and monitoring annual service plans, ensuring compliance with funding sources, and communicating with participants' families and staff.

What You Will Do

The DDS Case Manager will provide case management, assist participants in accessing community resources, develop and implement annual service plans, and ensure compliance with funding sources and CARF standards.

Requirements

  • Bachelor's degree in Human Services, Psychology or related field
  • Minimum 1 year of experience in case management
  • Current Connecticut Driver's License
  • Satisfactory motor vehicle check
  • Daily use of a reliable/insured vehicle
  • Willingness to obtain and maintain medication certification, CPR, and PMT certifications
