Requirements

  • Dentist license with the State of Washington
  • DEA controlled substances registration
  • Basic Life Support for Healthcare Providers (CPR/AED) certification
  • Verified immunity to measles, mumps, rubella (MMR), and varicella (chickenpox)
  • Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) or equivalent
  • High school graduate or equivalent
