Role Overview

The Diabetes Care and Education Specialist (DCES) provides direct care, education, and support to individuals with diabetes, helping them reach personalized goals related to nutrition, behavior, and treatment. The DCES works closely with managing clinicians to deliver integrated, person-centered care and serves as a resource for the broader care team.

What You Will Do

The DCES will assess care needs, develop and implement care plans, and provide education and support to patients and caregivers. They will also collaborate with the care team to ensure continuity of care and promote quality diabetes management.

Requirements

  • Bachelor's degree in Nursing or Nutrition and/or Dietetics
  • 2 years of prior experience in diabetes care and education
  • Current national certification as a Certified Diabetes Care & Education Specialist (CDCES)
  • Current BLS certification for health professionals
  • Effective communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong clinical judgment and critical thinking skills
  • Ability to assess patient needs and develop individualized care plans
