Requirements

  • Provide individualized care as outlined in the individual’s Life Plan/ IPOP.
  • Assist individuals in the primary activities of daily living (toileting, feeding, cooking, and cleaning) as outlined in the individual’s Life Plan / IPOP to promote independence.
  • Maintain files and produce service notes and documentation on assigned caseload.
