Requirements

  • Supervise Direct Support Professionals
  • Oversee the day-to-day operation of the program
  • Provide direct support
  • Manage individual records and billing
  • Assist with program planning and implementation
  • Maintain licensing compliance
  • Manage individual assets
  • Complete incident reports
  • Compile census and attendance information
  • Review and attest to accuracy
  • Submit accurate data to the Program Director
  • Schedule and assist with health care appointments
  • Administer or support self-administration of medication
  • Purchase and maintain medical supplies and equipment
  • Develop menus and purchase food and supplies
  • Direct and monitor food service
  • Maintain productive and professional relations with individuals, families, case managers, and other stakeholders
  • Implement Sevita Customer Service Standards
  • Train DSPs in delivery
  • Assist with recruitment and hiring
  • Populate and maintain schedule
  • Deliver orientation and ongoing training
  • Prepare and conduct annual evaluations
  • Coach employees as needed
  • May remove employees in the event of emergency
  • May recommend and participate in decisions regarding termination
  • Consult with the Program Director on all matters of discipline
  • Responsible for employee safety and workers' compensation
  • May assist with or lead monthly program meetings
  • Documents monthly meeting agenda and attendance
  • Reviews census, attendance information or other required documentation to assure accurate billing
  • Monitors utilization between authorization and provision of services and supports
  • Monitors staffing hours for budgetary compliance
  • Captures daily census within systems
  • Escalates potential service disruptions
  • Supports location preparations required for expected placements
  • Maintains neat, clean and safe environment for individuals served
  • Makes or arranges for minor repairs
  • Reports to and schedules repairs by maintenance personnel as required
  • Monitors environmental safety
  • Monitors water temperature, refrigerator and freezer temperature, and safety alarms and extinguishers on a monthly basis
  • Replaces furnace filters monthly
  • Performs other duties and activities as required
