Job Description
We are seeking a Discharge Planner RN or LPN to work with our team to coordinate patient transitions and discharges. The ideal candidate will have 2 years of clinical experience in a psychiatric setting and a current State of Alabama License RN, LPN, or MSW.

Requirements

  • Two years of clinical experience in a psychiatric setting
  • Medical terminology is essential to this position
  • Licensure: RN, LPN, or MSW with psychiatric nursing experience preferred
  • Current CPR and CPI Certification required
  • Demonstrates proficiency with EMR Software Applications
